FORMS REQUIRED TO PARTICIPATE IN SPORTS
1. Seattle School District Athletic Registration & Physical Form; 2. The Specific Sport Sheet;
3. Concussion Form ; 4. The LAST PAGE of the Student Handbook ; 5. Drivers Consent;
6.GHS Agreement; 7. Emergency Form
